Description:
Radiologists
and hospitals frequently employ physician extenders,
non-physician professionals who assist the radiologist
and perform certain imaging services independently.
These professionals may have a variety of different
credentials, including Radiology Practitioner Assistant
(RPA), Radiologist Assistant (RA), Physician Assistant
(PA), Nurse Practitioner (NP), and others.
This session will briefly discuss the differences among
the various credentials in terms of training and education.
Then we will review in depth the Medicare requirements
for non-physician practitioner billing and for “incident-to”
billing, including the types of professionals to which
these concepts apply; the limitations and restrictions
on place of service, type of service, etc.; and the
type of physician supervision required. Examples will
be provided of billable and non-billable services performed
by the various types of professionals in the hospital
and non-hospital setting (imaging center, private office).
Time will be reserved at the end of the audio-web conference
to answer participants’ questions about material
covered in the session.

The Goal:
The
goal of this session is to prepare attendees to accurately
assign diagnosis codes for diagnostic radiology services,
discuss the differences between coding for commercial
payors and Medicare, review common coding errors and
discuss the quality of clinical information and the
operational processes that may impact the coding process.
Objectives:

new and revised ICD-9-CM diagnosis codes for 2009
relevant to radiology.

Review official coding guidelines for diagnosis
coding for diagnostic radiology services

Review common coding “tips and traps”
to identify and address potential coding errors

Discuss the source and quality of clinical information
utilized for diagnosis code assignment.
Abstract:
Official
coding guidelines for diagnosis code assignment for
diagnostic radiology services will be presented in an
easy to understand format. How Medicare may differ from
other payors in their requirements and how the pre-certification
process for commercial payors will be reviewed to ensure
compliance.
What is the correct code assignment for an infiltrate?
A breast nodule? Common modality specific coding tips
and traps will be reviewed in detail to ensure correct
coding practices.
Can diagnosis information be obtained from other sources
if it is not included in the radiology report? What
do you do if the clinical information provided in the
report header does not relate to the exam being performed?
When must you call the referring physician’s office
to obtain additional information? These questions and
more will be answered during the portion of the conference
dedicated to reviewing the quality and source of clinical
data utilized in the coding process. Operational processes
that affect the quality of diagnosis coding will be
reviewed to equip the attendee with the information
needed to review their own organization and create a
plan of action to fix systematic problems!

Diagnosis Coding 2009"

This session
will include an overview of the Official ICD-9-CM Diagnosis
Coding Guidelines as maintained by the American Hospital
Association. In addition to code definition and sequencing,
specific codes for neoplasms, prophylactic therapy,
“history of” and supportive care will be
discussed.
Through the use of specific examples, this session will
prepare participants to accurately report documented
medical conditions and ensure appropriate reimbursement.
A single diagnosis code may not accurately report the
patient’s medical status, and this session will
emphasize the need to report all conditions managed
by the medical or radiation oncologist.
Establishing medical necessity is a concern that affects
all healthcare entities, and oncology specialties are
not immune to this problem. This presentation will include
a brief discussion of the Medicare definition of medical
necessity, and the documentation to support diagnosis
code assignment that establishes the need for patient
services.

to Radiation Oncology"

services provided during a course of radiation therapy
have unique codes with potentially complex definitions.

oncology, and review the translation of these terms

who need a high-level introduction to radiation oncology

of therapy.

Practitioners in Medical Oncology"

Description:
This session
will include common definitions of nonphysician practitioners,
services typically provided by these individuals, and
current utilization in the field of medical oncology.
Existing urban legends regarding new patient medical
conditions, shared visits and more will be discussed.
The Centers for Medicare and Medicaid Services (CMS)
has published numerous documents regarding the use of
NPPs in an ‘incident to’ capacity, and the
Office of the Inspector General (OIG) has targeted ‘incident
to’ billing for review in calendar year 2008.
It is also important to remember that ‘incident
to’ is a Medicare convention only, and other payors
may or may not subscribe to this billing methodology.
The decision to hire a nonphysician practitioner generally
requires significant front-end planning, and tips will
be provided in this session to help the oncology practice
analyze the impact of a nonphysician on practice revenue.
